My license is expired. How do I reinstate it?
Contact the Board Office to request information on how to reinstate your expired license. When you contact the Board, you will be asked to provide your name and any changes, your current mailing address and a phone number where you can be contacted. Holders of expired licenses are required to submit documentation of CEUs, resumes with work histories, verification of licenses held in other states as well as the renewal application form, questionnaire, and a check or money order for renewal and late fees. How do I get a verification/certification of my license sent to another state or entity?
Submit your request in writing to the Board; include your name, license number and the name and address of where you would like the verification/certification sent. The fee for a verification/certification is $15; submit a check or money payable to the Commonwealth of Massachusetts with your request.

I have not received my renewal application. How can I renew my license?
If your license has not yet expired, contact the Board Office at 617-973-0806 to obtain a renewal application. When you contact the Board, you will be asked to provide your name, any changes, your current mailing address and a phone number where you can be contacted. If your license expiration date has passed, see #1 above.
