Attention Primary Stroke Service (PSS) Designation Applicants: In an effort to process the Primary Stroke Service designation applications in a timely manner and assist in Emergency Medical Services planning, the Department is asking hospitals that intend to apply for PSS designation to electronically submit a 'notice of intent'. Such notice of intent should include an estimated date that the hospital will be ready for an on-site survey. The notice of intent should be submitted by email as soon as possible to Gail Palmeri at Gail.Palmeri@state.ma.us. A completed application must subsequently be submitted to the Department.
To request voluntary designation as a provider of Primary Stroke Services, hospitals must:
- Complete the Primary Stroke Service Licensure Application;
attach comments on additional sheets of paper if necessary;
- If Primary Stroke Services are to be provided at multiple campuses of the hospital, complete a separate application for each campus;
- Return the completed application to:

After review of the application, a Department representative will call the hospital contact person identified on the application to discuss the application and arrange for a site visit. Documents identified on the Primary Stroke Service Licensure Application Materials List must be readily available for on-site review by survey staff.
Further information about the data management vendor will be available on the Division of Health Care Facility Licensure and Certification website within the next few weeks.
