In accordance with M.G.L. c. 111L, §10(i), an institution holding a Certificate of Registration to conduct human embryonic stem cell research shall submit an annual report to the Department of Public Health.

Due Date

Annual Reports are due on January 31 immediately following the reporting period.

Reporting Period

An institution's annual report should cover the period from January 1 of the reporting year or from the date of issuance of the first certificate of registration (if issued after January 1) through December 31 of the reporting year.

Report Content

Annual reports should provide a summary of the research approved to the extent required by M.G.L. c. 111L, § 3(b). The report should also include a statement representing that said research was reviewed in accordance with M.G.L. c. 111L, if applicable.
