Instructions for Access
To utilize the faxed response option, simply fill out the attached form, Nurse Aide Inquiry Response System User Profile , and indicate that you wish to receive your inquiry responses via fax. Once the form is completed, send it to our office using one of the methods detailed on the form. If you wish to forego using the form, you may include all of the requested information in an e-mail to our Registry Data Coordinator at JIM.HUGG@STATE.MA.US.
After we have received your information, we will update your facility file and send you confirmation of the requested change(s). You should begin receiving faxes the same night that the change was executed in our system.
If for any reason, we are unable to fax your inquiry responses, our system will default to the surface mail posts as a temporary measure. These printed responses will be posted to you on the next business day. In this way, we can ensure that you will receive the requested information in a timely manner.
While we have endeavored to create an error free system, there are still some potential problems you may encounter while using the fax option. The majority of these problems are as a result of fax machine incompatibility with our data modem. Should you discover that you are not receiving faxed responses as expected, please contact Registry staff at 617-753-8144 to report the problem. When you call, please have technical specifications for your fax available so that we are better able to assist you.
