Active Tuberculosis Disease
If you have a patient that you strongly suspect may have active TB disease or you have received laboratory confirmation of Active TB disease in one of your patients you must immediately fill out the Active TB reporting form accurately and completely and fax it to the number indicated on the form.

If you need emergency assistance reporting a case of Active TB disease outside of normal business hours please contact the Epidemiologist on duty at (617) 983-6800.
Latent TB Infection
If you are a provider that has administered a Tuberculin Skin Test (TST) or an Interferon Gamma Release Assay (IGRA) where the result was positive, you must complete a Latent Tuberculosis Infection reporting form according to the attached instructions and fax it to the number indicated on the form.
