The Soldiers' Home may use medical information about you to provide you with medical treatment or services. We may disclose medical information about you to physicians, nurses, technicians, or other personnel of the Soldiers' Home in Chelsea or contracted personnel of the Soldiers' Home in Chelsea who are involved in taking care of you either as an inpatient or outpatient. For example, the physician treating you for a heart condition may need to know if you have diabetes because of the effects diabetes may have on you. In addition, the physician may need to tell the dietician that you are diabetic in order that your meals as an inpatient may be arranged. Different departments of the Soldiers' Home in Chelsea may also share medical information about you in order to co-ordinate the different services you may need, such as prescriptions, lab work, x-rays and therapy. The Soldiers' Home may also disclose medical information about you to people outside this facility who may be involved in your medical care such as family members, health care proxy, or others we use to provide services that are part of your care.
The Soldiers' Home may use and disclose medical information about you so that the treatment and services you receive may be billed and payment may be collected from you, your insurance carrier or a third party. For example, the Soldiers' Home may need to give your health plan information about services you received so that your health plan will pay us or reimburse you. The Soldiers' Home will not use or disclose medical information to contact you as a reminder that you have an appointment for treatment or medical care at out facility unless you have specifically authorized us, in writing, to so do.
The Soldiers' Home may include certain limited information about you in the directory while you are an inpatient at our facility. This information may include your name, location within the Soldiers' Home, your general condition and your religious affiliation. This information, excluding your religious affiliation, may also be released to a person who asks for you by name. Your religious affiliation may be given to a member of the clergy such as a priest, minister or rabbi, even if they don't ask for you by name. You have a right to further limit this information and if your request is in writing, we will abide by your request until such time as we are otherwise notified. You may decide not to have your name in the directory. If you decide you do not want your name in the directory, the request must be in writing.
Individuals Involved in Your Care or Payment for Your Care:
The Soldiers' Home may release medical information about you to a friend or family member whom you have designated as your health care proxy or you have authorized us to release medical information. We may also tell your family or friends your general condition and your location in the Soldiers' Home in Chelsea . If you decide you do not want anyone to know this information, you must do so in writing. The Soldiers' Home may also disclose medical information about you to an entity assisting in disaster relief efforts, so that your family can be notified about your condition, status and location.
As Required by Law:
The Soldiers' Home will disclose medical information about you when so required to do by federal, state or local law.
To Avert a Serious Threat to Health or Safety:
The Soldiers' Home may use and disclose medical information about you when necessary to prevent a serious threat to your health, and safety or the health and safety of the public or another person. Any disclosure, however, would be to someone able to help prevent the threat.
