Patrick Administration Announces $14.5M to Hospitals and Health Centers to Provide More Effective and Cost-Efficient Care

BOSTON — The Patrick Administration today announced $14.5 million in funding to 57 hospitals across the state, including critical access hospitals and community health centers, to help them transition to integrated delivery systems that provide more effective and cost-efficient care to patients in need.

"We rely on hospitals and community health centers throughout the Commonwealth to provide residents with accessible, cost-effective health care," said Governor Deval Patrick. "This funding will help providers improve services so patients get the care they need, and ultimately, it will support our top priority of controlling health care costs."

“This funding will help provide more integrated care and outreach to vulnerable populations that will result in better health outcomes in the long run,” said Secretary of Health and Human Services John Polanowicz. “I thank the participating hospitals and health centers for their commitment to advancing our cost-containment goals.”

Funding was included in the FY13 state budget and the awards are supported by federal matching funds. The funding is designed to support one of five priority areas at hospitals and community health centers. Grantees applied for, and received funding to support one of the following:

  1. Developing fully integrated delivery systems.
    1. Medical home transformation
    2. Integrating physical health and behavioral health
    3. Developing integrated acute and post-acute networks
  2. Developing alternatives to fee-for-service payments that promote system sustainability.
    1. Develop capacities to accept global or alternative payments
    2. Enhance data analytics and quality performance measurement capabilities
  3. Improving health outcomes and quality.
    1. Establish a chronic disease registry
    2. Implement system of complex care management for high risk populations
    3. Redirect non-emergent Emergency Dept. use to Community Health Centers
    4. Reduce variations in inpatient care for patients with high-risk conditions
  4. Enhancing outreach and enrollment. Design, implement, and document enrollment, outreach and health care access projects for individuals who may be eligible for public subsidized and non-subsidized health insurance programs and who may require individualized support due to geography, ethnicity, race, culture, immigration, disability, or disease status.
  5. Building strategic business and operations capacity. Provide detailed plan of consulting services to be employed to improve strategic planning and conduct assessments of operations to make the provision of care to MassHealth members more effective and cost-efficient.
