HSPC participates in the Diabetes Prevention and Control led, Massachusetts Health Disparities Collaborative. This is an effort to achieve strategic system change in the delivery of primary health care. Community health centers across the Commonwealth participate in a chronic disease collaborative, modeled after the US Department of Health and Human Services Health Disparities Collaborative, and will serve as an equivalency collaborative for federally funded health centers. Participating health centers will be required to track core measures on people with chronic disease as systems changes are implemented in the areas of self-management, decision support, clinical information systems, delivery system design, and health care organization.
