Eligibility dates are not printed on a member's card. You must use EVS to verify the member's eligibility using the Provider Online Service Center (POSC) every day you provide service.
- The cardholder's number appears below the cardholder's name.
- The back of the card has been revised to simplify language, and to add the TTY number (for the deaf and hard of hearing) for the member Customer Service: 1-800-497-4648
