Error 0282 - The Number of Days Is Missing.

Error: The number of days is either missing or invalid.

Resolution: If the number of days has been entered, ensure they were calculated correctly.

Note: If the patient status code indicates "still a patient," count the number of days from the first date of service (DOS) through the last date of service. If the patient status code indicates the patient was "discharged or transferred," do not count the last DOS when calculating the number of days. For example, if billing 12/1/08 through 12/15/08 and the patient status indicates that he or she is still a patient, enter 15 as the number of days; if billing 12/1/08 through 12/15/08 and the patient status indicates that he or she was discharged or transferred, enter 14 as the number of days.

Error 0283 - The Number of Days is Invalid.

Resolution: Electronic Claim Submission: Correct claim by providing the date of other payer issued on the explanation of benefits (EOB).

Paper Claim Submission: Ensure that the EOB date is legible on any attachments when submitting on paper.

Error 2526 - Zero TPL Amount And No Adj Rsn Code - Header

Error: The other payer amount is zero or blank, and there is no valid adjustment reason code present at the claim header.

Resolution: Electronic Claim Submission: If the other payer paid amount is greater than zero, provide information and enter the correct adjustment reason code ( ARC) at claim header. If the other payer amount is zero, enter the correct ARC at claim header.

Paper Claim Submission: Ensure that the Other Payer Paid Amount and ARC are present and legible on all attachments when submitting on paper

Error 2527 - Zero TPL Amount And No Adj Rsn Code - Detail

Error: The other payer amount is zero or blank, and there is no valid adjustment reason code present at the claim detail.

Resolution: Electronic Claim Submission: If the other payer paid amount is greater than zero, provide information and enter the correct adjustment reason code (ARC) at claim detail. If the other payer amount is zero, enter the correct ARC at claim detail.

Paper Claim Submission: Ensure that the Other Payer Paid Amount and ARC are present and legible on all attachments when submitting on paper.

Error 0503 - Date Dispense After Billing Date (Pharmacy only Edit-inactive for all claim types except for CTP)

Error: There is a discrepancy between the explanation of benefits (EOB) from the other insurer and the MassHealth claim, or in the other information from the primary insurer.

Resolution: Verify and correct the claim or EOB (as necessary) then resubmit the claim. Refer to the provider billing tips flyer, Correcting Error Code 4203, located in the Billing Tips link under MassHealth Customer Service for Providers in the Information for MassHealth Providers section on www.mass.gov/masshealth for assistance in correcting the issue.

Error 2614 - Managed Care Service Should Be Paid by RMC

Error: The mental-health or substance-abuse treatment service entered on the claim must be billed to the Massachusetts Behavioral Health Partnership.

Resolution: To avoid this error, use EVS to verify coverage type listed in category 35 (MassHealth Substance Abuse).
