Workplace Disease & Injury Reporting

    Health care providers are required to report occupational injuries and diseases to Occupational Health Surveillance Program (OHSP) as required by the state law.

  • Reporting Occupational Fatalities

    OHSP maintains a 24-hour hotline to facilitate reporting of occupational fatalities.
