The following policy applies only to the use of the Department of Public Health MassCHIP Web site. Web sites operated by other state agencies have different policies. Information Voluntarily Provided by You.
This site collected voluntary information from users through the e-mails that you sent through this site, and via the on-line submission forms in use until February 2, 2009. Through on-line registration forms, users provided the MassCHIP system with basic contact information (name, address, telephone number, FAX number and e-mail address) and type of organizational affiliation selected from a checklist provided by the registration site. These data are not currently being collected as of April 8, 2009. MassCHIP has used these data to communicate with users (e.g., inform them of new releases of the system), to analyze utilization patterns in order to make the system more responsive, and to insure compliance with the confidentiality standards stated in the user agreement. If patterns of utilization by an individual user are detected that suggest that an attempt is being made to identify an individual, the user is contacted and cautioned that such attempts will result in loss of access to MassCHIP. Registration information is also used to determine if there are relationships between classes of users and the reports they access. E-mails sent by you to this site and information submitted via on-line submission forms will contain personally identifiable information.
