Attention All DFS Licenseholders
As of May 30, 2008, if you change your address and hold an approval, license, certificate, or permit from the Department of Fire Services, you have 14 days to notify the Office of the State Fire Marshal, according to new state regulations. If you hold a license from the Office of the State Fire Marshal and have recently changed your address, please complete the change of address form  and mail it to the address listed below.

If you want to add an additional restriction to your MA Certificate of Competency please complete the add an additional restriction form  and mail it to the Licensing Desk Address below.

If you want to apply for an additional restriction to your company's MA Certificate of Registration, please complete the application to add a restriction to your company Certificate of Registration  and mail it to the Licensing Desk address below.

To apply for a retest please complete the application for retest form  and mail it to the Licensing Desk address below.
