Effective January 25 th:
The Department of Public Safety ("Department") is pleased to announce that effective January 25, 2008, the following amendments to 520 CMR 5.00 (Amusement Devices) went in to effect:
For owners of amusement devices (520 CMR 5.01-5.12)
- Certified Insurance inspectors are now required to provide notice of any changes in their insurance coverage to the Department and must provide proof of insurance upon relocation or new employment;
- Owners are now required to attest to the fact that they will comply with the CORI procedure submitted to the Department.
- Owners are now required to submit an attestation to the Department that they have complied with all manufacturer safety requirements/recommendations for modification.
- Owners are now required to submit manuals for devices being licensed in electronic format (if manual is not already on file with the Department).
- The Certified Maintenance Mechanic (CMM) is now required to sign off on a Department issued checklist after their inspection.
- All Certifications issued by the Department to the CMM will expire on their birthdate.
- Owners are now required to notify Department of a change to the CMM.
For Owners of Amusement Devices/Challenge Courses/Climbing Walls (520 CMR 5.14-5.15):
- Owners are now required to attest to the fact that they will comply with the CORI procedure submitted to the Department.
- Site Plans/Floor Plans are required to include sequential numbering of elements starting at 01.
- Owners must now maintain records that new staff (since licensure) have been properly trained and notify Department of certain staffing changes.
- Certified Insurance inspectors are now required to provide notice of any changes to their insurance coverage to the Department and shall provide proof of insurance upon relocation or new employment;
