If you have a disability that substantially limits your ability to perform a major life activity, as defined under the Americans with Disabilities Act ("ADA"), and you require accommodations on an examination administered by the Department of Public Safety ("Department"), you 1) must complete the Request for Accommodations Form , available at the link below, and 2) must submit documentation from a licensed professional confirming your diagnosis and need for accommodations.
