State Police Investigate Fatal Pedestrian Crash on Route 291 West in Springfield
On November 19, 2007 at approximately 6:18 PM, Troopers assigned to the State Police Barracks in Springfield responded to a report of a motor vehicle crash involving a pedestrian on Route 291 West at Exit 2B in Springfield that resulted in one fatality.
Preliminary investigation by Trooper Jarred Tiberi indicates that Perez was walking from the median onto the westbound travel lanes of Route 291, and was struck in the left travel lane by a 1990 Buick LeSabre operated by Thomas Mazza, age 58 of Agawam, MA. Perez was subsequently struck by a 2005 Mazda sedan operated by Melissa Domingos, age 26 of Chicopee, MA, a 2000 Dodge Intrepid operated by Theresa Black, age 58 of Westfield, MA, a 2002 Mercury sedan operated by Maryellen Kissel, age 52 of Springfield, MA, and a 2001 Chevrolet Cavalier operated by Anne Kelly, age 48 of Holyoke, MA. Perez sustained fatal injuries and was declared deceased at the scene.
This crash remains under investigation with the assistance of the State Police Collision Analysis and Reconstruction Section, the State Police Crime Scene Services Section and the State Police Detective Unit assigned to Hampden County. The Springfield Police and Fire Departments and MassHighway assisted Troopers at the scene.
Due to the investigation the ramp from Route 291 West to Route 91 North was closed for approximately four hours.
