Preliminary investigation by Trooper Nathan Cinelli indicates that a 2002 Ford pickup truck operated by 48-year-old Kevin M. Smith of Wales, was traveling eastbound on Route 20 near Little Alum Road when the Ford crossed over into the westbound lanes of Route 20 striking a 1993 Geo Prism sedan head-on operated by 24-year-old Nicholas Perry of Holland. Smith was transported with serious injuries to Harrington Memorial Hospital in Southbridge and subsequently to UMass Memorial Medical Center in Worcester.

State Police Detectives from the Hampden County District Attorney's Office, the Sturbridge Police Department, Brimfield Fire Department & Ambulance, the Office of the Chief Medical Examiner and MassHighways assisted Troopers on scene.

Due to the crash investigation and vehicle removal, the left eastbound travel lane and both westbound travel lanes of Route 20 were closed for approximately 3 1/2 hours.
