This document provides an outline of the process criminal justice agencies and their vendors must follow to become approved to use hardware and/or software on the CJIS network.

This template is completed and submitted by criminal justice agencies and their vendors who wish to become approved to use hardware and/or software on the CJIS network. It is also used to request/report changes over time, such as requests to implement new transactions and reports of vendor legal name changes. - FBI Security Addendum
This document identified the duties and responsibilities of individuals and agencies so that the security and integrity of the DCJIS and FBI information resources are not compromised. Vendors and their personnel are required to sign this agreement before gaining access to the CJIS network.

- Vendor Agreement of Non-Disclosure
This document is used by criminal justice agencies working with a vendor that will have access to criminal justice information. An authorized representative of both the vendor organization and the criminal justice agency is required to sign this agreement before any vendor personnel can be granted access to the CJIS network.
- Individual Agreement of Non-Disclosure (AOND)
All vendor personnel with access to criminal justice information are required to sign this agreement before gaining access to the CJIS network.
- Vendor Personnel List
Used by criminal justice agencies to track and report on all vendor personnel working with the agency who have/had access to criminal justice information and/or the CJIS network, including initial personnel and personnel joining the project over time. The template includes key information about each person, including information on background record checks, signed agreements, security trainings, and more.
- Vendor VPN User Application
Used by vendors who have been approved to develop CJIS functionality, this template is used to request access to the CJIS network via a secure Virtual Private Network (VPN) connection.
- CJIS Broker Authorization Form
Used by criminal justice agencies and their vendors who have been approved to develop CJIS functionality, this template is used to request authorization to access the CJIS Broker web service via the CJIS network. Information provided on this form will be used to authenticate and authorize requests and responses to and from the CJIS Broker.
- Improper Access and/or Dissemination of CJIS Information Complaint Form file size 1MB
Used to report any potential improper access to, and/or dissemination of, criminal justice information. The template includes information about what CJIS information was accessed, when, and by whom.
