(also known as Triage)

Overview: The inter-agency, multi-disciplinary committee established within each institution is chaired by the Reentry Liaison and meets monthly to develop an individual reentry plan for each inmate prior to release to the community. The role of this committee is to review the following factors as they pertain to each offender: housing plan, medical & mental health continuity of care and substance abuse treatment, employment and transportation arrangements. Each committee member begins coordination of the release planning efforts 6 months prior to release utilizing Inmate Management System (IMS). The assigned Correctional Program Officer (CPO) presents information about the needs of each offender to the Institution Reentry Committee who works to identify programs/linkages to address the identified need areas.
