His Excellency Governor Deval L. Patrick
Whereas The number of Massachusetts citizens who possess health care proxies is low, resulting in patients not receiving the type of care they would want at the end of their lives; and
Whereas The Massachusetts Medical Society, The Hospice and Palliative Care Federation of Massachusetts, the Executive Office of Elder Affairs, The Department of Public Health, Health Care for All, and other organizations and individuals are working to increase the number of people who possess a health care proxy– a simple document designating a trusted relative or friend to make health care decisions for them if they are unable to make them for themselves; and
Whereas A health care proxy increases the likelihood of a person receiving the type of health care that is consistent with his or her beliefs and values; and
Whereas We urge Massachusetts citizens to discuss health care decision-making with a relative or friend, with the goal of creating a health care proxy,
Now, Therefore, I, Deval L. Patrick, Governor of the Commonwealth of Massachusetts, do hereby proclaim April 16th, 2012, to be,
National Healthcare Decisions Day
And urge all the citizens of the Commonwealth to take cognizance of this event and participate fittingly in its observance.
Given at the Executive Chamber in Boston, this first day of March, in the year two thousand and twelve, and of the Independence of the United States of America, the two hundred and thirty-fifth.
