Use this form if you are a purchasing official for a city, town, county, or authority within the Commonwealth and purchase goods or services from vendors on Massachusetts Operational Services Division (OSD) statewide contracts and contracts issued by an executive department, pursuant to 801 CMR 21.00 that is open for use by other entities (department contracts).
According to M.G.L. c. 30B, within 15 days of contract execution, a governmental body must forward a copy of any electricity or natural gas contract to which it is a party and a report of the process used to execute the contract to the Department of Public Utilities, the Department of Energy Resources, and the Office of the Inspector General. The Office of the Inspector General periodically issues requests for responses (RFRs), which are available at COMMBUYS, the official procurement record system for the Commonwealth of Massachusetts' Executive Departments's. Five forms, which must be submitted with an RFR response, are available by clicking on "Solicitations of the Office of the Inspector General".
Prior to using the construction management at risk (CM at risk) delivery method to procure a contract for the construction, reconstruction, installation, demolition, maintenance, or repair of any building estimated to cost $5 million or more, an awarding authority must obtain a Notice to Proceed to use the CM at risk delivery method from the Office of the Inspector General. Use this form to apply for a Notice to Proceed.
Prior to using the design-build delivery method to procure a contract for the construction, reconstruction, alteration, remodeling, or repair of any public works project estimated to cost $5 million or more, an awarding authority must obtain a Notice to Proceed to use the design-build delivery method from the Office of the Inspector General. Use this form to apply for the Notice to Proceed.
