“provided further, care management under section 3 of chapter 211 of the Acts of 2006 shall be implemented through Aging & Disability Resource Consortiums, which shall include a combination of one or more Aging Services Access Points and Independent Living Consortiums, and not less than $1,500,000 shall be used to implement the provisions of section 2 of Chapter 211 of the Acts of 2006, the pre-admission counseling and assessment program, so-called, through the development of said Aging & Disability Resource Consortiums;”
