AN ACT establishing the Massachusetts Vision Coalition Fund

Be it enacted by the Senate and House of Representatives in General Court assembled, and by the authority of the same, as follows:

SECTION 1.    Chapter 90 of General Laws, as appearing in appearing in the 2002 Official Edition, is hereby amended by inserting after section 8D the following new section:-

Section 8D1/2. “The application form for a renewal issuance or renewal extension shall include language permitting a voluntary contribution of $1 or more per applicant, to be quarterly distributed by the registrar to the Massachusetts Vision Coalition, a not-for-profit organization, to prevent blindness and preserve the sight of resident of this state. A statement providing an explanation of the purpose of the funds shall be included with the application form. Prior to the registrar distributing funds collected pursuant to paragraph 8D1/2, the Massachusetts Vision Coalition must submit a report to the registrar that identifies how such funds were used during the preceding year.
