Companies must submit the online application and fax pre-screening form(s) to 617-723-5594 for each new hire along with a current Department of Revenue (DOR) Certificate of Good Standing. Failure to provide a current Certificate of Good Standing will result in denial of funding.

  1. Complete application online.(an application contains up to 13 new hires)

  2. A pre-screening form pdf format of    HITG Pre-Screening Form  for each potential new hire, included in an application, must be submitted at the time of the application. Fax the form to 617-723-5594. As a preference for veterans, the 6-month or longer unemployment requirement is waived for qualified veterans (copy of DD214 is required)
  3. Department of Revenue (DOR) Certificate of Good Standing (see sample certificate pdf format of    Sample of Certificate of Good Standing  ) must be faxed - certificate must be less than 6 months old.

  4. Fax the completed pre-screening form(s) and Certificate of Good Standing to WTF at 617-723-5594.
