Companies must submit the online application and fax pre-screening form(s) to 617-723-5594 for each new hire along with a current Department of Revenue (DOR) Certificate of Good Standing. Failure to provide a current Certificate of Good Standing will result in denial of funding.

As a preference for veterans, the 6-month or longer unemployment requirement is waived for qualified veterans (copy of DD214 is required) - Department of Revenue (DOR) Certificate of Good Standing (see

) must be faxed - certificate must be less than 6 months old.
