Persons wishing to file a complaint against the business practices of any employment agency must complete the form below and attach any supporting documentation available to support his/her claim(s). Complaints are not accepted verbally, over the telephone, or via fax or e-mail. A complainant must complete the form below, and mail or deliver the form to DLS.

Only complaints that demonstrate violation of a provision(s) of the Employment Agency Statute will be investigated by the Employment Agency Program. Complainants will be notified about the status of their complaint. Licensees/agency owners about which a complaint has been filed will be contacted by the Employment Agency Program, and an inspection of agency records will occur. When a hearing is required, the licensee (agency owner) is required to attend and will be provided ample time to respond to the complaint; complainants are also required to attend the hearing to present his/her case and evidence to support the claim(s). Hearings are not open to the public.

Please note: DLS does not regulate quality of care provided by caregiver services. The Employment Agency Program regulates the employment and service agency industry in Massachusetts for the purpose of worker protection.
