If you receive a "Notice of Disqualification", a request for a hearing must be made within 10 days after the date the notice is issued.

A late request for a hearing may be allowed if it is filed within a 30-day period after the disqualification notice was issued, and if you can establish that there was "good cause" for the delayed request. If your late request is denied, you may appeal that denial.  If your late request is eventually approved, a hearing will then be scheduled on your original disqualification.
