A party may appeal a decision issued by the DUA Hearings Department by filing an application for review with the Board of Review within 30 days of the date of the DUA Hearings Department decision.

An appeal may be filed using UI Online, submitting the request that was included with your decision or by completing the attached Form 1801 pdf format of 1801.pdf .  Request can be submitted by mail, fax, or hand deliver it to the Board of Review at:

An application for review submitted through the U.S. mail will be deemed to be filed timely if the U.S. postmark date falls within 30 calendar days of the date on the hearings decision.

An application for review sent through any private courier or hand delivered must be received at the Board of Review office before 5:00 p.m. on the last day of the 30 calendar day appeal period.

An application for review submitted via fax will be timely if received at the Board of Review office before 5:00 p.m. on the last day of the 30 calendar day appeal period.
