1. Scroll to bottom of screen
- Is someone listed in the Review/Select Owner/Officer Info section?
- If YES,
- Select the Radio button next to the Owner/Officer Name
- Select the Modify button
- The Owner/Officer information will populate the fields above
- Complete the required fields
- If YES,
- If the Owner/Officer is an individual, enter only the:
- First Name
- Middle Initial
- Last Name (No special characters)
- SSN (No Dashes)
- Compensation Question Yes/No
- The Legal Entity Name and FEIN fields MUST BE BLANK for an individual
- Complete the Additional Information section
- NO DASHES OR SPECIAL CHARACTERS except in date field (MM/DD/YYYY or MM/DD/YY)
- Select Save
- Address Validation Screen appears, Select Correct Address, Select Next
- Owner/Officer screen appears Scroll down to see updated list of Owner/Officers in Review/Select Owner/Officer Information section
- Select Next to complete Activation
- If the Owner/Officer is a Legal Entity, enter only the
- Legal Entity Name
- FEIN (No Dashes)
- Compensation Question Yes/No
- First Name, Middle, Last Name, SSN MUST BE BLANK
- Complete the Additional Information section
- NO DASHES OR SPECIAL CHARACTERS except in date field (MM/DD/YYYY or MM/DD/YY)
- Select Save
- Address Validation Screen appears, Select Correct Address, Select Next
- Owner/Officer screen appears Scroll down to see updated list of Owner/Officers in Review/Select Owner/Officer Information section
- Select Next to complete Activation
- If NO, go to step 2
2. Enter the New Owner/Officer information.
- If the Owner/Officer is an individual, enter only the:
- First Name
- Middle Initial
- Last Name (No special characters)
- SSN (No Dashes)
- Compensation Question Yes/No
- The Legal Entity Name and FEIN fields MUST BE BLANK for an individual
- Complete the Additional Information section
- NO DASHES OR SPECIAL CHARACTERS except in date field (MM/DD/YYYY or MM/DD/YY)
- Select Add
- Address Validation Screen appears, Select Correct Address, Select Next
- Owner/Officer screen appears Scroll down to see updated list of Owner/Officers in Review/Select Owner/Officer Information section
- Select Next to complete Activation
- If the Owner/Officer is a Legal Entity, enter only the
- Legal Entity Name
- FEIN (No Dashes)
- Compensation Question Yes/No
- First Name, Middle, Last Name, SSN MUST BE BLANK
- Complete the Additional Information section
- NO DASHES OR SPECIAL CHARACTERS except in date field (MM/DD/YYYY or MM/DD/YY)
- Select Save
- Address Validation Screen appears, Select Correct Address, Select Next
- Owner/Officer screen appears Scroll down to see updated list of Owner/Officers in Review/Select Owner/Officer Information section
- Select Next to complete Activation
3. Delete Owner/Officers that are duplicate or are no longer with the
employer.
- Select the Radio button next to the Owner/Officer Name
- Select the Delete button
- Owner/Officer screen will refresh and the deleted Owner/Officer should no longer appear in the list at the bottom of the screen.
- Other things to look for:
- No special characters (, . % -) except in date field (MM/DD/YYYY or MM/DD/YY)
- Ownership cannot equal more than 100%
