A party may appeal a decision issued by the DUA Hearings Department by filing an application for review with the Board of Review within 30 days of the date of the DUA Hearings Department decision.

To file an appeal with the Board of Review, complete the attached Form 1801 pdf format of 1801.pdf and mail, fax, or hand deliver it to the Board of Review at:

An application for review submitted through the U.S. mail will be deemed to be filed timely if the U.S. postmark date falls within 30 calendar days of the date on the hearings decision.

Applications for review sent through any private courier or hand delivered must be received at the Board of Review office before 5:00 p.m. on the last day of the 30 calendar day appeal period.

Applications for review submitted via fax will be timely if received at the Board of Review office before 5:00 p.m. on the last day of the 30 calendar day appeal period.
