The relevant medical records must accompany all claims in which liability is contested. At the time a claim is filed, the insurer must provide the claimant, or claimant's representative, with any records that contain any history of the injury for which treatment is being sought, or that contain opinions by medical personnel as to the nature and causation of the injury or condition being treated. 452 CMR § 1.07(2).
