Provider Application
You are able to apply for recertification as a designated provider of vocational rehabilitation services under the Massachusetts workers' compensation act.A renewal letter will be sent out to you from the department's Office of Education and Vocational Rehabilitation (OEVR), dated the first Monday in March of the current year, with attachments for you to fill out and return. All information must be received by OEVR no later than the last Friday in April. When submitting your recertification information, all providers are required to include their e-mail address on all correspondence including their resume. In addition, any company employing several vocational rehabilitation specialists are also required to indicate the specialist's e-mail address on their resume. The Department of Industrial Accidents has converted to an electronic document management system and all information will be sent into OEVR via e-mail.-
