Form 132 - Affidavit in Support of Employee's Request for Speedy Conference Because of Hardship

These forms are used by the employee's attorney, or the employee if he/she does not have an attorney, to request an earlier date for a conference, due to a severe hardship. This form cannot be filed until after the case has been conciliated.
