In Massachusetts, police officers who arrest a defendant for OUI-alcohol will take him to the barracks or police station and offer him a breath test as part of the booking procedure.  If the defendant refuses the test, or if he consents and fails the test, the officer will record that fact in the breath test device.  That information is then electronically transferred to the Registry of Motor Vehicles and triggers the appropriate license suspension.
