To determine if you are eligible to take a refund, please answer these questions:

1) Are you presently receiving Workers’ Compensation payments,
either on a weekly basis or, in the case of a lump-sum
settlement, during the period of time over which the
lump-sum settlement is allocated?

2) Do you currently have EITHER a pending claim for Workers’
Compensation OR a pending application for retirement (this includes superannuation retirement, under the “regular” or RetirementPlus plan, as well as ordinary or accidental disability retirement)?
Yes No
3)

Are you on a paid or unpaid leave of absence?

Yes No
4) Will you be accepting employment with a public school system
or any other political subdivision which requires membership
in a Massachusetts contributory retirement system?

5)

If your employment has been involuntarily terminated, are
you appealing—or do you intend to appeal—your termination?

6) Do you believe that you EITHER have 20 years of creditable service, OR have 10 years of creditable service and are age 55 or older? Yes No
