Interactive version for payroll administrators
Substitute, temporary or part-time service purchase application

Go to Substitute, temporary or part-time service purchase application
(Part 2 only)
(2 pages, pdf)

What is this?

  • This is Part 2 of our Substitute, temporary or part-time service purchase form (the section that employers must complete).
  • Unlike the Part 2 that appears in the downloadable application that is accessible to members, it is interactive, meaning that you can complete it onscreen.

You are welcome either to complete the copy of Part 2 that members provide or to access the interactive version on this page. Either way you choose to complete your part is fine; the advantage of the online interactive version is that you may more easily type in your data. In all cases, the completed form must be printed out and signed. It is not possible for the applicant or you to submit the form either by e-mail or via the web.

Access the member application process and complete form:
For your reference, a guide to the substitute, temporary or part-time service purchase process—which contains an interactive eligibility “quiz,” general information on what it means to purchase substitute, temporary or part-time service, and access to the application—is available to members at http://mass.gov/mtrs/2members/20active/20purchase/20sub.htm.

Just as you’ve done with this form in the past, please complete, sign and return Part 2 directly to the member. It is then the applicant’s responsibility to submit both Parts 1 and 2 of the application to the MTRS.

  • As you probably know, however, you will NOT be able to “save” the file with any data that you enter; once you close the document, the fields revert to blank or the default settings.

Accordingly, please be sure to PRINT and PROOFREAD your document BEFORE you close it, as you cannot save your data.
