For benefit recipients (retirees and survivors)

All of these forms are in portable document format (pdf). This file format allows you to view and print an MTRS form in its original layout by using Adobe Acrobat Reader. Please note:

  • At this time, these forms cannot be submitted to us electronically (this is to ensure your security and because we need to have your original signature). To use a form, print it on letter-size white paper, complete it and then mail it to us at the address noted on the form.

For active members

1) Refund application
(For withdrawing or rolling over your MTRS account)
If you leave active teaching service and wish to withdraw your funds, please walk through this step-by-step process which will guide you to the appropriate downloadable application (total refund to you, or rollover to an eligible plan).

Retirement applications

 
  When you’re ready to retire, access our online benefit estimator and review the general retirement process. At the end of that process, you’ll be able to download a retirement application.
  If you should find yourself permanently disabled and unable to work for an extended period of time, please review the process for applying for a disability retirement benefit.
3) Beneficiary Designation—Active Member (2 pages) Use this to name a beneficiary or to change your beneficiary designation. For more information, see also Survivor Benefits.
4) Direct Rollover Acknowledgement Form (2 pages; for service purchase payments only) If you are purchasing creditable service and wish to pay for your service by way of a direct rollover, use this form. For more information, see also Purchasing creditable service with a direct transfer or rollover.
5) Trust-to-Trust Transfer Acknowledgement Form (2 pages; for service purchase payments only) If you are purchasing creditable service and wish to pay for your service by way of a transfer, use this form. For more information, see also Purchasing creditable service with a direct transfer or rollover.
6) Disability application All applications for disability retirement must be reviewed by the members of the Board at one of their monthly meetings. Only the Board may grant disability retirement benefits.

For benefit recipients (retirees and survivors)

1) Direct Deposit Authorization (2 pages) Use this to receive your payment via direct deposit to your bank account instead of by mailed check, or to change the bank account to which your benefit payment is directly deposited. For more information, see Direct deposit information.
2) Change of Address—Retired Member (1 page) Use this to notify us of a temporary or permanent change in your address.
3) Substitute Form W-4P
(1 page)
Use this to change your federal tax withholding. Please send your completed form to our main office: MTRS, One Charles Park, Cambridge, MA  02142-1206.  
4) Retired member/Option B only: Beneficiary Designation Form (2 pages) For Option B retirees ONLY: If you wish to change your Option B beneficiary, complete and send us this form.
