1 Effective July 22, 2002 Aetna U.S. Healthcare Inc. changed its name to Aetna Health Inc.
2 Blue Cross and Blue Shield of Massachusetts, Inc. ("BCBSMA") applied for a company license for HMO Blue, to be known as Blue Cross and Blue Shield of Massachusetts HMO Blue, Inc. This license, effective December 1, 2004, was created as a separate corporate entity for BCBCMA's HMO line of business. HMO Blue was formerly a line of business of Blue Cross and Blue Shield of Massachusetts, Inc.
3 Harvard Pilgrim Health Care of New England, Inc. is a Massachusetts domiciled HMO that does not currently offer products in Massachusetts.
4 Tufts Associated Health Maintenance Organization, Inc. has filed the necessary business documents to do business as "Tufts Health Plan."
5 Effective July 17, 2002 United HealthCare of New England, Inc. changed its name to UnitedHealthcare of New England, Inc. On June 2, 2011 UHCNE notified the Division of Insurance that it intends to discontinue and nonrenew all of its health insurance business in Massachusetts and eventually withdraw its license to operate as a Massachusetts Health Maintenance Organization.
