⁴ Bankers Life and Casualty Company notified the Division that these long-term care policies are no longer being marketed in Massachusetts.
⁵ Effective December 29, 2011 Berkshire Life Insurance Company of America notified the Division that it will cease offering the approved individual long term care policy in Massachusetts.
⁶ Effective March 31, 2005, Catholic Order of Foresters notified the Division that it ceased offering these approved individual long-term care policies in Massachusetts
⁷ Effective October 3, 2003, Continental Casualty Company notified the Division that it will no longer be offering these approved long-term care policies in Massachusetts.
⁸ Effective December 31, 2007, CUNA Mutual Life Insurance Company merged into CUNA Mutual Insurance Society.
⁹ Farmer's New World Insurance Company notified the Division that it has not marketed these long-term care policies in Massachusetts.
¹⁰ Fidelity Security Life Insurance Company notified the Division that it has not marketed its approved long-term care policy in Massachusetts. If offered, the company has an approved home health care rider it will offer with the above-listed nursing home only policy.

On August 24, 2006 Transamerica notified the Division that it will begin offering its previously discontinued plan [policy form TOL 1-FP (MA) 402].
All other plans listed continue to no longer be offered in Massachusetts.
