Purpose:

To clarify the requirements for funeral establishment name changes.

Policy:

In order to change the name of a funeral establishment a registered licensed funeral director (Type 3) registered to the establishment must file an application for a new establishment certificate. The following information is required to be provided with the completed funeral establishment certificate application: Business or trade name prior to and after the name change. The applicant will be notified by the Board if a board appearance is required.
