Licensee Databases

The Division of Professional Licensure is pleased to offer databases containing licensee information that is considered public record. This process allows us to quickly provide you with the detailed, accurate information you need in a user friendly and efficient format.

The Division will provide certain public information in accordance with the Public Records Law MGL c4, s7; MGL c66, s10 and 950 CMR 32.00.
Only the following public information about active licensees licensees will be provided:
- Board Name
- License Type
- License Number
- Licensee Name
- Licensee Mailing Address
- License Issue Date
- License Expiration Date
- License Status
A CD-Rom containing a fully functional application will be mailed to customers for a fee ($30 per selected box in the
request form 
). The application consists of an executable file and the database(s) you requested. Each database contains all of the current records for the chosen profession(s). Important notice regarding the DPH Health Professional Boards and the Licensee Databases.
The Division does not have the capability to provide a Macintosh compatible version of this application and cannot provide support to Macintosh users. Macintosh users will need to seek third party assistance to work with the application. However, some newer models of MacIntosh computers are able to run Microsoft Windows type applications. Check with your MacIntosh support professional to determine if your MacIntosh computer can be used to run the application we send you.
Ordering the Database(s)
To place an order please click on the link below to complete and print the Request for Licensee Database Form. Mail your completed form along with your payment to the address at the top of the form.
Important Notice:
Please note that beginning November 3, 2008, the Division of Professional Licensure will no longer provide Licensee Databases for the following Professions:
- Dental Hygienists
- Dentists
- Drug Stores/Pharmacies
- Licensed Practical Nurses
- Nursing Home Administrators
- Perfusionists
- Pharmacists
- Registered or Advanced Practice Nurses
- Respiratory Care Specialists
- Wholesale Distributors
