This is a full-featured software application based on Microsoft Access 2000/XP. Included in these .zip compressed files is a professional setup program.
Important Note: if you have already installed the software from a CD-ROM from the DTC, do not install the following as it will wipe out your old data. This file is the same version of the CD-ROM software and not an upgrade. This downloadable version is provided for immediate use instead of using the mailed CD-ROM version.

This is a full-featured software application based on the Microsoft Access 2000/XP.
Instructions for using the Microsoft Access 2000/2003
Version of the Form 500 database
You will need to have Microsoft Access 2000, 2002, or 2003 installed on your PC in order to run this application
Open Windows Explorer
Use Windows Explorer to create a subfolder named " Form 500" within the " C:\Program Files" folder. Make sure there is a single space between the words "Form" and "500" or, the application will not work
Copy the files f500app2003.mdb
(Access 2002/2003 version),
f500app2000.mdb
(Access 2000 version), and
f500data2000.mdb
from this zip file into the new folder " C:\Program Files\Form 500"For future ease-of-use, create a shortcut on your desktop to: C:\Program Files\Form 500\f500app2003.mdb or f500app2000.mdb by right mouse clicking on your desktop and selecting the menu item "create shortcut" if you are using the earlier Access 2000 version of the software.
Label the icon Form 500 (optional)
If you receive an error message when you start the application, open the file called references.jpg
from within the zip file and set your Access references to be the same. Save and restart the database.Step 1:
Enter your operator information
Use the section on the lower left of the form to enter the towns you operate in. As you select each town from this list, fill in the subscriber subform to the right. Be sure to fill in all data fields.
Note that clicking on each town allows you to go back and change the town information on the right side. Clicking on the "Save data" button brings you back to the main formStep 2:
Enter complaints for each town
The arrow buttons will move backwards and forwards for each of the records you enter
Carefully note the onscreen prompts on the form for navigation of the records
Be sure to fill in all data fields
Make sure that the total number of complaints equals the value amount you placed in the "manner of resolution" section
Clicking on the "Save data" button brings you back to the main menu formStep 3:

Be sure to fill in all data fields
Clicking on the "Save data" button brings you back to the main menu formStep 4:
Use this button to create a data file to send to the Competition Division via CD, an email as a .ZIP file attachment or, print a report. If you choose to print a report, remember to select the correct year and town or the report will display incorrect information.
