PV278) CHINA STAR RESTAURANT – 1773 MIDDLESEX STREET – LOWELL

The Petitioner (Mr. James Chan) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) Table 1. The Petitioner notes the present occupancy is 180 persons. The men’s room has 3-water closets and 3-urinals. The women’s room has 3 water closets. The owner is seeking to increase the occupancy to 300 without add toilet facilities. The Lowell Board of Health concurs with the request. The Health Director sent the concurrence to the plumbing inspector and building commissioner and they had no objection to the request.

GV11) RITA SUPERMARKET – 515 LAWRENCE STREET - LOWELL

The Petitioner (Mr. Rodolfo Casado, Journeyman Plumber) requests a variance from 248 CMR (Code of Massachusetts Regulation) 3.00, the Uniform State Fuel Gas Code, Section 3.04 (1). The Petitioner seeks a waiver to install an unlisted gas steam table. As this is a gas variance, no Board of Health letter is required.

GV25) QD VISION – 29 HARTWELL AVENUE - LEXINGTON

The Petitioner (Mr. Jack Burroughs, Facilities Mgr.) requests a variance from 248 CMR (Code of Massachusetts Regulation) 3.00, the Uniform State Fuel Gas Code, Section 3.04 (1). The Petitioner seeks an approval to allow the use of a gas process machine at this location. As this is a gas variance, no Board of Health letter is required

PV26) LITTLE CAESAR’S PIZZA – NS COGESSHALL STREET – NEW BEDFORD

The Petitioner (Mr. Charles Fougere, Sr. Project Mgr.) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) Table 1. The Petitioner provided the Board Office with a copy of a petition letter sent to the New Bedford Board of Health.

GV30) CLOVER FAST FOOD, INC. – 1075 CAMBRIDGE STREET - CAMBRIDGE

The Petitioner (Mr. Ayr Muir, President/CEO) requests a variance from 248 CMR (Code of Massachusetts Regulation) 3.00, the Uniform State Fuel Gas Code, Section 3.04(1). The Petitioner seeks permission to allow the installation of a non-product accepted oven. The specialized of is manufacture in Israel. The petitioner included in the submittal, a copy of an ISO certification. As this is a gas variance, no Board of Health letter is required

PV33) TENANT RETAIL SPACE – 147-151 MASSACHUSETTS AVENUE - BOSTON

The Petitioner (Mr. Christopher Mulhern, Architect) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) (i). The petitioner proposes the installation of one unisex non-handicap employee restroom in lieu of the required two. The Petitioner provided the Board office with a copy of a petition letter sent to the Boston Board of Health.

PV36) TASTY BURGER – 69-71 L STREET – SOUTH BOSTON

The Petitioner (Mr. David Dubois, CEO) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) Table 1. The petitioner applied for a building permit and informed he needed a public restroom to accommodate the outdoor seating. The petitioner provided the Board Office with a copy of the petition letter sent to the Boston Board of Health.
