PV99) CHIPOTLE MEXICAN GRILLE-316 NORTH STREET – FANEUIL HALL - CHARLESTOWN
The Petitioner (Mr. Joseph Fiorino, Plumbing Designer) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10.10 (18) Table 1. The restaurant will occupy 1,263 square feet of space on both the first & second floors. Total seating one the first floor is 18, the second floor is 30. Outdoor seating is 28 and there are 10 employees. Total occupancy is 86. The Petitioner proposes installing male, female and employee restrooms on the 2nd floor only. The petitioner notes there is an elevator outside the entrance to the restaurant. The petitioner provided the Board office with a copy of the petition letter sent to Mr. Gary Moccia, Deputy Commissioner.
PV100) RAFEEQUE RESIDENCE – 75 HILLTOP ROAD – CHESTNUT HILL
The Petitioner (Mr. Rafeeque Bhadelia, Owner) requests a variance from 248 CMR (Code of Massachusetts Regulation) 3.00, the Uniform State Plumbing Code, Section 3.04 (1). The Petitioner purchased an onyx/marble sink at the Granite Auction and had it installed by his plumber. During his inspection, the Brookline inspector, Timothy Lynch advised Mr. Bhadelia the sink was not product accepted and Mr. Bhadelia should apply for a variance or remove the fixture. The Brookline Board of Health has no objection to the request for a waiver.
PV101) PATTY JOE’S PO-BOY CAFÉ – 1209 BEDFORD STREET - ABINGTON
The Petitioner (Mr. Arthur McKenna, Owner) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section .10.10.(18) Table 1. The petitioner is seeking a waiver from the requirement to install a service/mop sink in this establishment. Petitioner states it cutting the concrete floor creates a hardship for installing the sink. The petitioner states his business was at this location since 2007. It came to his attention that the Federal Food Code requires this fixture. The Board office received the petition letter sent to the Board of Health. The Board of Health voted to approve not installing the sink.
PV103) MATHWORKS – 4 APPLE HILL - NATICK
The Petitioner (Mr. Sebuh Petrosian, project manager) requests a variance from 248 CMR (Code of Massachusetts Regulation) 3.00 &10.00, the Uniform State Plumbing Code, Sections 3.04 (1) & 10.14 (5) (h). The Petitioner installed 19 butterfly valves at this location. The valves are not in compliance with 248 CMR. The Natick Board of Health has no objections to the applicant’s request.
PV105) TENANT UNITS – 7 SUMMER STREET – MANCHESTER BY THE SEA
The Petitioner (Mr. Remko Breuker (Agent) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) (i). The Petitioner proposes one unisex toilet room as core and common for two employee spaces, and employees from one space to access the toilet room through a shared space conference room. The Manchester Board of Health has no comment regarding this variance request.
PV109) ROCK HARBOR SEAFOOD – 18 OLD COLONY WAY – ORLEANS
The Petitioner (Mr. Christopher King (co-owner) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10. 06 (2) (o) 2. The project consists of removing existing floor drains and piping. Due to the corrosive nature of the salt water in the facility environment, the petitioner is requesting that the floor drains and piping be PVC. The Orleans Board of Health voted to approve the request.
PV111) MetroWest Regional Transit Authority – 15 Blandin Avenue - Framingham
The Petitioner (Mr. Ed Carr, Administrator) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code.. The Petitioner proposes erecting a 75x40 steel building for storage only of a loader, back hoe, bobcat and parking lot sander. As this is a state project no Board of Health letter is required.
PV112) UMASS AMHERST – CHENOWETH HALL – HOLDSWORTH WAY -AMHERST
The Petitioner (Mr. Peter Radzim, Chief Engineer) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.13 (L) (1) 5.. The Petitioner states the renovation of Labs 424, 426 & 430 include the addition of three hand-washing sinks with deck mounted eyewash stations. Petitioner is requesting permission to feed the hand washing sinks with tempered water from the new tempered water loop located on the fourth floor. As this is a state project no Board of Health is required.
