The Petitioner (Ms. Susan Varney, Administrative Assistant) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) Table 1. The petitioner proposes the installation of a unisex handicap accessible restroom in lieu of the required two. On October 10, 2012 the Phillipston Board of Health voted to approve this variance request..

PVC#92) CARISSA STEEFEL RESIDENCE – 483 BOYLSTON STREET #3 - BROOKLINE

The Petitioner (Ms. Carissa Steefel, owner) requests a variance from 248 CMR (Code of Massachusetts Regulation) 3.00, the Uniform State Plumbing Code, Section 3.04 (1). The petitioner purchased an Alden cast iron dual tub that is not product accepted. The Brookline Health Department acting as the Board of Health has no objection with this request.

The Petitioner (Paul Dionne, PJ Dionne Co., Inc.) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.09 (2) (c) 5 The petitioner notes a dishwasher was installed that did not discharge to a grease interceptor. Based on correspondence dated January 12, 2012 from the Melrose Board of Health, it appeared at that time issues were pending. Correspondence addressing the Board’s concerns was, sent on February 13, 2012.

The Petitioner (Mr. John Ferris, Business Director) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) Table 1. The petitioner is seeking relief from the fixture count requirements for the number of fixtures for the planned field project. The Hingham Board of Health Agent believes this is a valid request.’l/.

PVC#98) EDWARD KENNEDY INSTITUTE – 210 MORRISSEY BOULEVARD - BOSTON

The Petitioner (Blair Chamberlain, P.E.) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.16 (6) (a) 1. The petitioner is proposing to have plumbing vents vented in the areaway. The intent is to terminate the vents within the areaway attached to a wall and terminate at the plane of the roofline. As this is a State project, no Board of Health petition letter is required.

PVC#100) THE KING TUCK REALTY TRUST – 459 KING STREET - LITTLETON

The Petitioner (Mr. Charles Bush, MP) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) (i). Presently the space has one employee restroom. The petitioner is proposing to renovate the existing toilet room and convert it to a unisex handicap accessible restroom. The Littleton Board of Health has no objection with this variance request.

PVC#101) SRI LAKSHMI TEMPLE – 117 WAVERLY STREET - ASHLAND

The Petitioner (Mr. Daniel Junek, operations Mgr) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.06 (o) 3. The petitioner is seeking a waiver to install PVC piping for drains coming from 3 shrines that use a fruit juice type of liquid that has rotted out all of the existing cast iron pipe. The petitioner provided the office with a copy of the petition letter sent to the Ashland Board of Health.

PVC#103) ARCHSTONE – 2 LEIGHTON STREET - CAMBRIDGE

The Petitioner (Mr. Ralph Mulcahy, P.E.) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.06 (2) (g). The petitioner states the piping will be installed below a garage floor slab that is 3 stories below grade. The soil condition is corrosive to cast iron due to the chloride levels; cast iron would not last long in salt or brackish water. The petitioner proposes using HDPE piping to ensure longevity. The piping is SDR 11. The Cambridge Board of Health supports this request.

GVC#106) DWELLING – NICHOLAS NIGRO – 450 OCEAN STREET - MARSHFIELD

The Petitioner (Nicholas Nigro, Owner) requests a variance from 248 CMR (Code of Massachusetts Regulation) 3.00, the Uniform State Fuel Gas Code, Section 3.04 (1). The petitioner seeks a waiver to install a space heating & potable water system that is not Product Accepted. As this is a Gas Variance, no Board of Health letter is required.
