VARIANCE REQUESTS:

VR1) HOME DENTAL OFFICE - 251 SALISBURY STREET - WORCESTER

The Petitioner (Mr. Peter Stathoulo Poulos), Owner) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) (k) & Table 1 to allow the installation of one unisex handicap accessible restroom in lieu of the required two. The Worcester Board of Health, having concurrence from the Plumbing & Gas Inspection Unit of Inspectional Services, requests the variance.

VR2) CHUCK E. CHEESE - 132 SACK BOULEVARD - LEOMINSTER

The Petitioner (Mr. Ben Hughes P.E., Cahen Architectural Group) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) (b) 2.a to the installation of a new unisex handicap accessible family restroom in lieu of providing additional fixtures per Table I. The Leominster Board of Health and the Plumbing Inspector will honor the decision of the Plumbing Board.

VR3a) BUSINESS/MEDICAL SPACE - 1717 MAIN STREET - BROCKTON

The Petitioner (Heath Berkowitz, Owner) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) (k) and Table 1 to allow the installation of a unisex handicap accessible restroom in the event the space is leased for medical use. There are presently two existing employee restrooms and the unisex handicap is in addition to the existing. The Brockton Board of Health stands behind the request.

VR3b) BUSINESS/MEDICAL SPACE - 1717 MAIN STREET - BROCKTON

The Petitioner (Heath Berkowitz, Owner) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) (k) and Table 1 to not require the installation of a shower in the event the space is leased for medical use. The petitioner also requests he not be required to install a drinking fountain. A water dispenser will be provided. The Brockton Board of Health stands behind the request providing the tenants are aware of the variance.

VR4) VETERANS FIELD AT STONE PARK - 140 WHITING STREET - DEDHAM

The Petitioner (Mr. William Keegan, Town Administrator) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) Table 1 to allow the petitioners request toilet facilities be based on average attendance of school sporting events. The Dedham Board of Health is not opposed to the variance request.

VR5a) BERKSHIRE MEDICAL CENTER - 41 WAHCONAH STREET - PITTSFIELD

The Petitioner (Mr. Michael Passardi, VP Pittsfield Pipers) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) Table 1 to not install showers for this project. The Pittsfield Board of Health supports this variance request.

VR5b) BERKSHIRE MEDICAL CENTER - 42 SUMMER STREET - PITTSFIELD

The Petitioner (Mr. Michael Passardi, VP Pittsfield Pipers) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) Table 1 to not install showers for this project. The Pittsfield Board of Health supports this variance request.

VR6) DCR - HOUGHTON'S POND 848 HILLSIDE STREET - MILTON

The Petitioner (Mr. John Murray, Acting Commissioner) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.10 (18) Table 1 to not install showers for this project. As this is a state project, no Board of Health letter is required.

VR7a) DCAM - NEW LABORATORY SCIENCE BUILDING - UMASS AMHERST

The Petitioner (Mr. Ronald Ferrara, Project Manager) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Sections 10.07 (c) (1) and 10.08 (2) (g) to eliminate lead and oakum joints at the foundation wall. As this is a state project, no Board of Health letter is required.

VR7a) DCAM - NEW LABORATORY SCIENCE BUILDING - UMASS AMHERST

The Petitioner (Mr. Ronald Ferrara, Project Manager) requests a variance from 248 CMR (Code of Massachusetts Regulation) 10.00, the Uniform State Plumbing Code, Section 10.08 (2) (g) to not require cleanouts at the foundation wall on a 12" storm drain. The petitioner proposes to install manholes within close proximity of the foundation wall. As this is a state project, no Board of Health letter is required.
