Itemizing Campaign Finance Information
Receipts

Contributions from the same individual, committee or organization which in aggregate total more than $50 in a calendar year must be reported in a campaign finance report with the following information:

  • the contributor's full name;
  • the contributor's residential address (or committee address in the case of a state or local party committee, PAC or candidate's committee);
  • the date of the contribution;
  • the amount of the contribution;
  • the form of tender (cash, check, credit card, etc.);
  • for contributions of more than $200, the contributor's occupation and employer information;
  • for contributions from a state or local party committee, PAC or candidate's committee, the committee's CPF ID number;and
  • for contributions from a union or association, the name of one of the principal officers of the organization.
Expenditures

Expenditures that are $50 or more in a calendar year must be reported in a campaign finance report with the following information:

  • the vendor's (payee's) full name;
  • the vendor's address;
  • the date of the expenditure;
  • the amount of the expenditure;
  • the specific purpose of the expenditure; and
  • for expenditures made to other organized political committees, the committee's CPF ID number.
In-Kind Contributions

In-kind contributions from the same individual, committee or organization which in aggregate total more than $50 in a calendar year must be reported in a campaign finance report with the following information:

  • the contributor's full name;
  • the contributor's residential address (or committee address in the case of a state or local party committee, PAC or candidate's committee);
  • the date of the contribution;
  • the amount of the contribution;
  • a description of the goods or services contributed to the committee;
  • for contributions of more than $200, the contributor's occupation and employer information;
  • for contributions from a state or local party committee, PAC or candidate's committee, the committee's CPF ID number; and
  • for contributions from a union or association, the name of one of the principal officers of the organization.
Liabilities

All liabilities owed by the committee at the end of a reporting period must be reported in a campaign finance report with the following information:

  • the full name of the individual or business to whom the liability is owed.
    If the liability is a loan from the candidate to his or her committee, it should be indicated as such in the name field;
  • the address of the individual or business to whom the liability is owed;
  • the date the liability was incurred;
  • the amount of the liability; and
  • a description of the liability owed.
