As you were advised in PERAC Memorandum #37 of 2009, PERAC has issued an updated form entitled Application for Withdrawal of Accumulated Total Deductions. The attached CD (labeled September 2009) includes a PDF of the updated Withdrawal form with fill-in functionality. All of the other PDFs of forms on the attached CD are just as they were on the CD (labeled PERAC 2009) which was distributed by PERAC in March of 2009.
