Change of Address This form is to be used to inform the RMV of a change of address.
  1. Complete this form and mail it in an envelope to the following address or return it to any RMV branch office. RMV, Attn: Address Change, PO Box 55889, Boston, MA 02205-5889
  2. You will receive a letter of confirmation in the mail within seven (7) days. This letter will contain four (4) preprinted labels for you to attach to your license and registration(s).
  3. If you have more than one vehicle registered in your name, enter all plate numbers.
  4. Please make sure all information is correct and legible.
