Disabled Placard and Plate Form

Form	Description	Instructions
Application for Disabled Parking Placard/Plate (revised 09/06)	Special Placards and Plates are available for vehicles that transport medically disabled drivers and passengers.	The Application for Disabled Placard / Plate is a two page form. You are required to complete the first page of the form only. The second page of the form is to be completed by a Massachusetts licensed physician, chiropractor or nurse practitioner. After you complete the required information, take both pages of the form to your physician, chiropractor or nurse practitioner.
Report Handicap Parking Abuse (online submission)	Complete and submit this form to report suspected abuse of a disabled placard or handicap parking space.	This form may be completed and submitted electronically.
Report Handicap Parking Abuse (mail or fax)	Complete and submit this form to report suspected abuse of a disabled placard or handicap parking space.	Complete, sign, and mail or fax this form to the address or fax number located on the bottom of the abuseComplaint Form.
