This form is used to verify that your Massachusetts registered vehicle is "off the road" (such as in storage, or in the process of being restored/rebuilt) when due for a Massachusetts motor vehicle inspection, and you are unable to obtain a Massachusetts motor vehicle inspection for your vehicle. |
Complete the Off-the-Road Verification for Vehicle Inspection Form. Sign and date the form, mail it to the address at the top of the form, and keep a copy with your vehicle registration.
NOTE: If your vehicle is not actively registered in Massachusetts, you do not need to complete this form. |
Out-of-State Verification Inspection Form

This form is used to verify that your Massachusetts registered vehicle or equipment is located out of state when due for a Massachusetts motor vehicle inspection, and you are unable to obtain a Massachusetts motor vehicle inspection for your vehicle or equipment. |
Complete the Out-of-State Verification Inspection Form. Attach Proof of inspection from the state where the vehicle or equipment is currently located, if applicable, or certify that there is no motor vehicle inspection in the state where the vehicle or equipment is currently located. Sign and date the form, mail it to the address at the top of the form, and keep a copy with your vehicle or equipment registration. |
